The Somerset County Health Department is excited to announce the start of a program that will help address the diabetic needs of the county residents ages 18 and older. Through a federal grant, the Somerset County Health dept. will be offering the services of a registered Dietician, a Diabetes Educator, and glucose testing. For those clients that need help with medications and strips, this program will assist with free medications, strips, lancets, and glucometers (restrictions apply). Program Highlights:
Somerset County Health Department has joined with the National Diabetes Education program (NDEP), the American Diabetes Association (ADA), and locally with McCready Health Services Foundation, Atlantic General Hospital (AGH), and Three Lower Counties (TLC) to provide diabetes services among the Somerset and Worcester County. Appropriate diabetes management care allows the client to modify habits that might prevent complications in the future. The Somerset County Health Department offers a diabetes health clinic for the community.
The clinic is held every other Monday, 8:30 AM - Noon. Individual will receive American Diabetes Association (ADA) paper risk assessment test, blood pressure screening, along with an HbA1c and blood glucose test. Both blood tests are non-fasting and are finger-sticks.
A consultation with a registered dietitian and diabetes educator is available as well. For those clients that meet requirements and are in need of diabetes supplies and medications, this program can help too.
Qualifications: Live or work in Somerset and Worcester County.
