Closing the Gap on Diabetes Program
The Somerset County Health Department’ program will help address the diabetic needs of county residents ages 18 and older. Through a federal grant, the Somerset County Health dept. will be offering the services of a registered Dietician, a Diabetes Educator, and glucose testing. For those clients that need help with medications and strips, this program will assist with free medications, strips, lancets, and glucometers (restrictions apply). Additionally, we will provide self-management classes. For more information call the Somerset County Health Department at 443-523-1760.

Program Highlights:
Somerset County Health Department is partnering with McCready Health Services Foundation, Atlantic General Hospital (AGH), and Three Lower Counties (TLC) to provideify habits that might prevent complications in the futur diabetes services in Somerset and Worcester County. Appropriate diabetes management care allows the client to mode. The Somerset County Health Department offers a diabetes health clinic for the community.
Individual will receive American Diabetes Association (ADA) paper risk assessment test, blood pressure screening, along with finger stick for HbA1c and blood glucose test.
A consultation with a registered dietitian and diabetes educator is available as well. For those clients that meet requirements and are in need of diabetes supplies and medications, this program can help too.
Qualifications: Live or work in Somerset and Worcester County.
Open to all individuals 18 years and older
