Program Highlights:

The goal of the Cardiovascular Disease risk reduction program is to reduce disease, disability, and death related to CVD by :

1) Promoting healthy lifestyle behavior among individuals by teaching self-management skills.
2) Detecting and controlling risk factors by collaborative efforts with local medical facilities.
3) Creating healthy, supportive environments in families, schools, faith based organizations, and businesses.

SOME-R-SET to Get Heart Smart program is an educational campaign to raise awareness about the signs and symptoms of heart attack and stroke. The emphasis is on increasing physical activity and following a heart healthy diet. Walking programs are available which include incentives for participation.

Program Qualifications: Somerset County Resident

Schedule and Location: Ongoing Indoor Walking- Monday, Wednesday, and Friday from 8:30 am until10:00 am

Woodrow Wilson Community Center, Crisfield , MD 21817

No Charge
Must be 18 years or older to participate
Individual self-management education by appointment- Somerset County Health Department, 7920 Crisfield Highway, Westover , MD 21871
