Adult Evaluation & Review Services (AERS)

What is AERS? AERS provides assistance to aged and functionally disabled adults who are at risk of instituionalization. AERS staff conducts a comprehensive evaluation to identify services available to help the individual to remain in the community, or in the least restrictive environment, while functioning at the highest possible level of independence and personal well-being.
How does AERS help? AERS teams, located in local health departments, consist of a nurse, social worker, and as needed, a physician, psychiatrist and psychologist. The teams provide the individual with a comprehensive evaluation, including medical/nursing, environmental and psychosocial assessments. The evaluation is conducted in the home setting or the individual's current location. Upon completion of the evaluation, the multidisciplinary team develops a plan of care which recommends services needed to help the individual remain at home, or in the least restrictive environment. Every effort is made to obtain appropriate services to address the needs of each individual evaluated.

The Developmental Disabilities Program provides resource coordination to individuals with developmental disorders and their families. Our staff provides assistance to clients in accessing evaluation, treatment, residential, respite, and rehabilitation services.
Migrant Program

The migrant population is a transient group. During the summer months, there are approximately 1100 farm workers and their family that settle in Somerset County. The Somerset County Health Department provides services such as family planning, pregnancy testing , screening and treatment for sexually transmitted disease , TB and other communicable disease services , immunizations , drug and alcohol counseling , prenatal education and post partum home visiting. Staff conducts daily camp visits to triage for health problems and provides linkage to medical care follow up services. Disease prevention and health promotion education is provided individually or in a group setting. Translators/interpreters are hired to meet the bilingual needs.
Eligible children are referred to WIC and the Maryland Children's Health Program .
