Babies Born Healthy Program
The goal of this program is to improve access to early prenatal care in an attempt to improve birth outcomes and reduce infant mortality within Somerset County.
Services include:
Registered Nurse Case Managers provide services in the home that focus on assisting participants to access health care and related services, practice healthy behaviors and utilize good parenting skills.
Qualifications:
Somerset County pregnant women and children under age one, enrolled in the Maryland Medical Assistance Program, with a qualifying risk factor. |

Children With Special Health Care Needs Program
Somerset County Health Department has a special grant that can provide summer camp scholarships to children with special health care needs.
Funds will be available for possible assistance for the Summer of 2007. Please contact Sharon B. Corry, RN at 443-523-1800 for more information and camp scholarship applications.
Lead Poisoning Prevention
Provides extensive community outreach and education on lead. Also, case management services are provided to children 6 and under diagnosed with lead poisoning.

Also, information about lead is made available at such places as the county libraries, post offices, medical offices, etc.
Lead Case Management
Provides nursing services related to case management for all children under the age of 6 who have been diagnosed with lead poisoning . The nurse provides extensive education to the parents regarding lead poisoning and services including but not limited to, coordination of medical care, follow-up on lead levels, assistance with housing, nutritional teachings, etc.
