Mental Health Case Management

Offering adult, child, and adolescent mental health case management services to those in need of community resources and continuing support.
Services include providing home visits, transporting clients, connecting clients with resources in the community, assuring that services are being provided, and offering on going support in an effort to ensure client compliance and maintain community living.
Who is eligible for Mental Health Case Management services?
Adults - The following is a list of factors that may make an individual eligible for the program.
- must have mental health diagnosis
- needs intensive services to remain in the community
- unemployed, unable to work, or has poor work history
- has a poor personal social support system
- requires daily living assistance
- has inappropriate social behavior
- is homeless
- leaving a psychiatric hospital or residential treatment facility
- history of several psychiatric admissions
- history of several incarcerations
- has multiple disabilities (mental retardation, substance abuse)
- on conditional release
Children and Adolescents - The following is a list of factors that may make an individual eligible for the program.
- must have a mental health diagnosis
- may display functional impairment that interferes with functioning in family, school or community activities
- may be at risk of needing more intense level of care
- leaving a hospital or residential treatment center
- has past hospitalizations
- history of medication non-compliance
- in danger of out of home placement
- has multiple disabilities
- having difficulty accessing outpatient services
