Birth Certificates - $32.00
If you were born in the State of Maryland and we have a record of your birth certificate on file,  it is possible to get a certified copy from the Health Department.  Birth certificates are issued to the person or a parent and a photo ID is required as well as basic information about the birth.  It is recommended that you contact us to verify that your record is on file before coming in.

Death Certificates
You may apply to obtain a certified copy of a death certificate for deaths occurring within Somerset County.  The certificates are available for only 30 days following our receipt of the original death certificate.  You may contact us for fees and additional information.

Financial Interview & Fee Information

We accept Medical Assistance and some private insurance.  If you do not have insurance or assistance, a sliding fee scale is used to make treatment affordable for everyone who needs it and is based on your income.  Your fee, if any, will be determined when proof of income is provided to the Health Department.

There will be a $20.00 processing fee for all checks returned or not honored by the bank.
